The FDNY dispatch frequencies are not repeaters in the traditional sense, instead it's a full duplex system with voted receivers and a mixer.  The received audio from the mobiles/portables is injected into the audio transmitted by the base station when the mixer is turned on.  The mixer can be turned on and off by the dispatcher, and you generally won't hear the initial call from the mobiles/portables until the mixer is turned on.  If you want to hear "mixer off messages", you need to monitor the mobile frequencies and be close enough to the scene in order to pick them up.

The VHF frequencies will not be narrowbanded, however the FCC has granted the city a two-year narrowband wavier for these frequencies, with a deadline of December 31, 2014.

There is generally no PL transmitted on 160.280 for Hatzolah NYC.  A 167.9 PL is used on the same frequency for Sullivan County Hatzolah.  It's best to monitor the frequency with no PL to ensure you hear all transmissions.
